To register for a DBS check through St Margaret’s, please complete this form. This is only for members of the PCC.

Completing this form means that you consent to receiving a link from ‘Disclosure’ at Thirty-one Eight. Follow the instructions in the email to complete the check. We will also send you a Confidential Declaration form to complete.
